Equipment Loan Request & Agreement Form
Loan Requirements:
Equipment cannot be left unattended in a classrooom or a vehicle.
Students requesting to loan equipment must have already met with Dr. Matuszak about the request.
Any damage to equipment must be reported immediately.
The borrower is financially liable for damaged, misplaced, stolen, or lost equipment.
Equipment must be returned in the same condition in which loaned.
Equipment must be returned at the end of the same class period when used.
Only the student borrowing the equipment can use it.
For laptop loans in the Criminalistics Learning Lab, students must complete the equipment loan form before obtaining the laptop.
For laptop loans, files cannot be saved to the laptops and students cannot download files or software onto the laptops.
